This poster provides a summary of the work I completed on the Healthy Klamath Connect project while doing an internship in the Health Equity Department at Cascade Health Alliance (CHA), Klamath County's local CCO and administrator of the Oregon Health Plan (OHP). Healthy Klamath Connect is an online interactive resource guide and social resource referral system that CHA worked with Findhelp to develop for Klamath County.
